Philips has identified a problem with the M5072A SMART paediatric Defibrillation, used with the Philips HeartStart OnSite and HeartStart Home AEDs. The image on the cover of the cassette depicts electrode placement on an adult, rather than a child.

Stryker has found that some Physio-Control Lifepak AED devices have incorrect network settings. Medisol has not sold any affected AEDs.

Philips has identified a problem with the electrodes pads for the Philips HS1 AED, this problem may pose a hazard to patients or users.  This affects both M5071A adult pads and M5072A paediatric electrode pads. It has been observed that the gel of the pads detaches from the foam/foil backing when removed from the yellow plastic liner.

Defibrillators that are installed with software version V1.41 or lower for the CU-SP1 and V1.0 or lower for the CU-SP1 AUTO and are set to either English (British), Hungarian or Ukrainian language pack have been identified as having software issues.
